GLEN, the Gay and Lesbian Equality Network, is to close down.
It follows a review of the organisation which was commissioned after a series of scandals.
One of the allegations included financial mismanagement and oversight.
But Independent Consultant Jillian Van Turnhout says there was a string of problems including the cutting of funding and loss of staff, which led to a 'perfect storm', ultimately resulting in the decision to wind up the operation.
The controversy began when it emerged in April that the executive director Aine Duggan had reported 'grave concerns' to the charities regulator after she took over the top job.
Despite co-operating fully with the regulator the board of directors felt 'a perception of mistrust' had damaged GLEN.
